We are on a search for two Support / Case Workers in the Swansea area, with experience of housing, tenancy and mental health support.
- 1 x Part time - 28 hours per week - Fixed term to March 2025
These are floating support roles, working with individuals in their own homes across the Swansea area.
What do you get?

- Salary of £23,088 per year - pro-rata for part time
- 45p per mile travel expenses for work related mileage

- Working hours between 9am and 5pm - Monday to Friday
What will you do?
- You will lead on the assessment of needs for individuals referred to the projects.
- Establish a individualised support plan for each person (ISP).
- Work collaboratively with other relevant organisations e.g. CMHTs, the Local Authority, housing providers etc.
- Involve people we support, in the development, running and evaluation of the service.
- Liaise with other agencies with and on behalf of people we support as part of long term and crisis intervention support, including participation in joint meetings and case reviews.
- Assist people we support, to access information and advice.
- Provide resettlement support when moves to new accommodation take place.
- Keep accurate notes of all support provided.
What do we need?
- Someone with experience of supporting those who struggle with their mental health and wellbeing
- A Support / Case Worker with an excellent understanding of housing / homelessness.
- Knowledge of trauma informed practice, attachment theory and relationship-based approaches.
- A good understanding of safeguarding vulnerable adults.
- Someone who is self-aware and willing to engage in reflective practice.
- The ability to work with multi-agencies effectively.
- Highly organised, resilient, compassionate and non-judgemental.
